Creating smaller role, less prominent Non-Player Characters (NPCs) is most definitely allowed, even encouraged, in Pax Inter Pares. We like to see the additional flavour that they allow the game.
However, if you create an NPC with a rather well-known family name, please consult any actual Players who might be in that family and make sure they are willing to allow such a thing. While their unwillingness may not necessarily rule out your NPC becoming a part of the game, it is only fair. Especially if your character is 'semi-fictional.' For example, everyone knows there were Julii Caesares, but if seomone made up a Julii Ahenobarbi family, which is non-historical, that would be alright, but you should make the player(s) of the same gens aware.
